Guiding Your Journey

A typical hospice journey begins with a prospective patient or family member calling Heart of Hospice for information. Early contact helps patients and families make informed decisions regarding future choices. And obtaining all appropriate information about Heart of Hospice helps patients and families understand the role of hospice. If the family chooses the hospice care path, Heart of Hospice staff will contact the prospective patient's doctor to determine the medical appropriateness for hospice services. Or, quite often, the doctor or the hospital discharge planner will contact Heart of Hospice after they have discussed the hospice option with the patient and family. Then, as can be expected for medical care of any kind, a person becomes a Heart of Hospice patient by signing a consent form electing hospice care.
